Complete an application for membership form. Do not complete the Form of Nomination on the reverse of the form. A nomination is not necessary and may be made only after you have been admitted to membership.

 

Your pay number will be your account number.

Please complete your personal information clearly. Indicate where you are based. Your PPS number is essential.

 

You will need to provide photographic and documentary evidence of your name and address which must match the information on the membership form. It is acceptable to use your ID card and this can be placed on a recent utility bill, bank statement, etc and both photocopied on to one sheet. Bring this either to the credit union office or to one of the credit union authorised representatives in the different prisons.

Complete a payroll deduction form. When inserting the amount to be deducted please add €1 as there is a €1 entrance fee. Eg if you want €25 per fortnight deducted enter €26 on the form. Once the initial amount has been deducted it will then be reduced by the €1 to your regular amount. Leave the date of commencement blank and we will enter the next available date. If you wish to delay the commencement enter your preferred commencement date. You can change the amount at any time simply by telephoning the office.

Please give us your bank account details - 6 digit sorting code and 8 digit account number so that we can lodge your loan or withdrawal directly to your bank account.
